Breanne Muchemore

Vice President of Care Management

Breanne Muchemore is the Vice President of Care Management at Nova Leap Care Management. With more than 15 years of experience in healthcare and care coordination, she has dedicated her career to helping individuals and families navigate complex medical systems and access the support they need to live safely and independently.

Breanne’s passion for care management is rooted in her commitment to compassionate advocacy and practical solutions. She works closely with families, healthcare providers, and community partners to develop thoughtful care plans that prioritize dignity, quality of life, and peace of mind for those she serves.

Throughout her career, Breanne has led innovative programs that improve access to post-acute care and community resources and is widely respected for her collaborative leadership and patient-centered approach. She has served as a Board Member of the American Case Management Association (ACMA) since 2022. Breanne holds a Master of Social Work from Boston College and is a Licensed Independent Clinical Social Worker and Certified Case Manager.
