Breanne Muchemore is a nationally respected care management leader who helps individuals and families navigate aging, complex illnesses, and life-changing medical events. With nearly two decades of clinical and care management experience, she brings a rare combination of compassion, expertise, and strategic insight to Nova Leap’s private-pay care management services.
Breanne works with aging adults who wish to maintain independence and dignity, families caring for loved ones with chronic or serious conditions, and individuals recovering from catastrophic injury. Her approach is grounded in partnership, realistic planning, and whole-person advocacy, supporting families through decisions that are emotional, medical, and often overwhelming.
Before joining Nova Leap, Breanne spent more than a decade in senior leadership at Spaulding Rehabilitation Hospital, one of the most prestigious rehabilitation systems in the world. She supported some of the most medically complex patients in the country and developed care management models that helped families understand their options, advocate for their needs, and transition successfully to the next phase of recovery.
Breanne’s expertise extends beyond hospital walls. As a senior leader at Marquis Health, she oversaw post-acute referral pathways for one of the largest skilled nursing operators in New England. She partnered with hospital systems to create access to appropriate care, guide families through complex facility choices, and support patient safety as recovery continued beyond inpatient settings.
Breanne is a Licensed Independent Clinical Social Worker and Certified Case Manager. She earned her Master of Social Work from Boston College and her Bachelor of Social Work from Baylor University. She serves as President-Elect of the American Case Management Association (ACMA) Massachusetts Chapter, and lives north of Boston with her husband and 2 children.
