Support Throughout the Rehabilitation Journey

Families often contact a home-care agency only when discharge is approaching. At Nova Leap Home Health Southwest Florida, we can become involved sooner.

Our support may include:

• Companionship and reassurance during the rehabilitation stay 
• One-on-one assistance and supervision, as permitted by the facility 
• Communication with authorized family members 
• Participation in discharge-planning conversations when requested 
• Evaluation of expected care needs at home 
• Coordination of caregivers, nurses, or therapists before discharge 
• Home-safety and equipment-planning discussions 
• Transportation and assistance on the day of discharge 
• Increased support during the first days or weeks at home 

Physical Therapy

Physical therapy focuses on movement, strength, balance, mobility, and physical function.

• Walking and mobility
• Strength and endurance
• Balance and fall-risk reduction
• Transfers between the bed, chair, toilet, or other surfaces
• Use of walkers, canes, or other mobility equipment
• Recovery after surgery, injury, hospitalization, or illness
• Exercises and functional activities prescribed by the therapist

Occupational Therapy

Occupational therapy helps clients participate more safely and independently in the everyday activities that matter to them.

• Bathing, dressing, grooming, and toileting

• Safe use of adaptive equipment 
• Home-safety recommendations 
• Cognitive strategies and routines

Speech-Language Therapy

Speech-language therapy may help individuals experiencing changes in communication, cognition, voice, or swallowing.

• Swallowing concerns
• Strategies for communicating with family and caregivers
• Education related to safe eating, drinking, or communication techniques

Therapy Combined with Personal Home Care

A therapist may visit for scheduled treatment, but the client may still need help throughout the rest of the day. Our Care Partners can reinforce a safe daily routine and assist with personal care, meals, mobility, transportation, and other activities between therapy visits. This combined approach can help families avoid piecing together multiple disconnected providers.

Who May Benefit?

Therapy or rehabilitation support may be appropriate for individuals who are:

• Recovering from a hospitalization or surgery 
• Returning home from a skilled nursing or rehabilitation facility
