A Parkinson’s Foundation Community Partner

This national program educates and prepares home care and senior living staff to provide informed, compassionate support to people living with Parkinson’s disease. The curriculum focuses on practical strategies that can make a meaningful difference in everyday care, including medication timing, mobility support, individualized routines, and understanding how Parkinson’s symptoms can vary from person to person.

Parkinson’s Training for Our Care Team

Every newly hired Nova Leap Home Health Southwest Florida care partner is required to complete the Parkinson’s Foundation’s Essentials of Parkinson’s Care in Senior Living Communities and Home Care Agencies training before beginning work.

Through this training, our team learns to:

• Recognize both motor and non-motor symptoms 
• Understand the importance of consistent medication timing 
• Support mobility and daily activities safely 
• Respond patiently to changes in movement, communication, mood, and cognition 
• Adapt care to the individual rather than treating Parkinson’s as a one-size-fits-all condition 
• Communicate concerns and changes to the appropriate members of the care team 

Personalized Support

Parkinson’s affects every person differently. Some clients may need occasional support with errands or meal preparation, while others may require assistance throughout the day, overnight supervision, or around-the-clock care.

Depending on the client’s needs, our services may include:

• Assistance with bathing, dressing, grooming, and toileting 
• Mobility, transfer, and walking assistance 
• Fall-risk awareness and home-safety support 
• Medication reminders and support maintaining time-sensitive routines 
• Meal preparation and hydration reminders 
• Transportation to physician, therapy, and community appointments 
• Support following home exercise or therapy routines as directed 
• Assistance with communication and daily activities 
• Companionship and meaningful engagement 
• Overnight care 
• Respite care for family caregivers 
• Extended-hour or 24/7 home care 
• Skilled nursing services when clinically appropriate

Registered Nurse Oversight 

Assessments and individualized care plans are developed under the direction of a Registered Nurse and monitored as the client’s condition and needs change. Clients, families, and Care Partners also have access to 24/7 clinical support through our Director of Nursing, who is a Registered Nurse.

This allows changes such as increased falls, difficulty transferring, medication concerns, changes in appetite, confusion, sleep disruption, or declining ability to complete daily activities to be identified and communicated promptly.
