Nova Leap Home Health – Southwest Florida is proud to be a member of the Parkinson’s Foundation Community Partners in Parkinson’s Care program.
This national program educates and prepares home care and senior living staff to provide informed, compassionate support to people living with Parkinson’s disease. The curriculum focuses on practical strategies that can make a meaningful difference in everyday care, including medication timing, mobility support, individualized routines, and understanding how Parkinson’s symptoms can vary from person to person.
Every newly hired Nova Leap Home Health Southwest Florida care partner is required to complete the Parkinson’s Foundation’s Essentials of Parkinson’s Care in Senior Living Communities and Home Care Agencies training before beginning work.
Through this training, our team learns to:
Parkinson’s affects every person differently. Some clients may need occasional support with errands or meal preparation, while others may require assistance throughout the day, overnight supervision, or around-the-clock care.
Depending on the client’s needs, our services may include:
Assessments and individualized care plans are developed under the direction of a Registered Nurse and monitored as the client’s condition and needs change. Clients, families, and Care Partners also have access to 24/7 clinical support through our Director of Nursing, who is a Registered Nurse.
This allows changes such as increased falls, difficulty transferring, medication concerns, changes in appetite, confusion, sleep disruption, or declining ability to complete daily activities to be identified and communicated promptly.
